Port Stephens Peer Support Program

Port Stephens Peer Support is a service for younger persons with a disability and their carers, which offers social support through organised activities. Between eight to ten activities are offered per month. The activities are generally held after hours and on weekends.

The Peer Support Program assists to develop the person with a disability’s independence by providing social and recreational activities within the community.

Peer Support offers a break for families that care for a person who has a disability.

Who can receive care?

People aged between 16 and 64 who have a disability with low to moderate support needs and live in the Port Stephens Local Government Area can apply.

What are the costs involved?

The Peer Support Program is funded through Ageing, Disability and Home Care, Department of Human Services (ADHC), Home and Community Care Program (HACC). Clients will be asked to contribute towards the cost of transport and for those activities in which they participate. If costs are a problem, alternative arrangements may be made by contacting the Coordinator.

What activities are available?

Most leisure activities are available. Examples include: Movie Outings, Ten-Pin Bowling, Craft Activities, Horticulture/ Gardening, Attending Knights Football Games, Disney on Ice, Aerobics, Dinner/ Dance, Festivals, Shows etc. Clients are encouraged to provide feedback to staff, volunteers and the Coordinator regarding the activities and to contribute their ideas regarding what activities they would like to see offered.

Referrals

Referrals for the Peer Support Program can be made by self, relatives, carers, friends, Doctors, Social Workers, Discharge Planners, Allied Health Professionals and other Service Providers. All referrals for Peer Support are made through the Home and Community Care Access Point by contacting 1300 731 556.
